Education: Pharm-D. Experience: At least 3-year prior work experience including 1 year working in oncology section.. Job Responsibilities : 1. Ensure timely dispensing, dilution, and distribution of chemotherapy medications while reviewing patient profiles and CPOE to prevent drug interactions and promote rational drug use. 2. Verify patient profiles and baseline lab results (CBC, LFT, Creatinine) before preparing chemotherapy protocols, generating three copies (for nursing, patient, and pharmacy). 3. Double-check manual prescription forms and ensure they are signed before issuing and preparing chemotherapy drugs. 4. Issue intrathecal drugs (e.g., Methotrexate, Cytarabine, Hydrocortisone) early to prevent errors. 5. Confirm correct diagnosis, age, and chemotherapy protocol/phase before preparing the regimen. 6. Counter-check and verify all chemotherapy drugs before final preparation and handover to nursing staff. 7. Maintain hard copy records of chemotherapy issuance to ensure patient rights and document any unadministered or refunded drugs. 8. Contact the physician for any discrepancies in medication orders and document the intervention. If unresolved, share with the clinical team for review by the formulary committee. 9. If medications are unavailable, contact the physician to arrange for alternatives. If no substitute is available, process a Non-Formulary (NFI) arrangement with the required signed approval. 10. Monitor inventory levels, report discrepancies or shortages to Pharmacy Management, and prepare requisitions for the warehouse. 11. Receive goods from the warehouse, verify batch numbers, expiry dates, quantities, packaging, and cold chain requirements. Ensure proper storage according to temperature and conditions. 12. Ensure light-sensitive, cytotoxic, high-alert, and look-alike/sound-alike drugs are stored properly, and monitor sterile areas for correct temperature, humidity, and pressure. 13. Provide drug information to healthcare professionals as needed and maintain records of the information provided. 14. Submit monthly reports on stock levels, detailing fast-moving, slow-moving, expired, broken, and damaged items. 15. Provide technical support and conduct training sessions for pharmacy staff. 16. Prepare and present Continuing Medical Education (CME) sessions on pharmaceutical topics. 17. Submit monthly reports on drug interventions and related information to the Manager/Deputy Manager. 18. Ensure preventive maintenance of pharmaceutical equipment (e.g., refrigerators, air conditioners, biological safety cabinets) is completed on schedule. Provide regular updates and KPI reports to management and complete any other tasks assigned by the Location Supervisor or Pharmacy Management.
