Education: Pharm-D (Doctor of Pharmacy), Advanced certification / formal training in Critical Care Pharmacy (preferred); PGY-1 Residency and/ or Board Certification in Critical Care (BCCCP) or in pharmacotherapy (BCPS) (preferred).. Experience: Minimum of 5 years of relevant hospital pharmacy experience (Without PGY-1) , including 2 years providing critical care / intensive care pharmaceutical care.. Job Summary: The Clinical Pharmacy Specialist provides expert, patient-centered pharmaceutical care to critically ill patients as an integral member of the interdisciplinary team, in accordance with institutional and international standards (SCCM/ACCP/ASHP Position Paper on Pharmacy Services. Through daily rounds, comprehensive medication management, protocol development, antimicrobial stewardship, emergency response, education, research, and quality improvement, the role advances the safe, effective, and cost-conscious use of medicines and optimizes outcomes for critically ill patients. Task & Responsibilities: Clinical Pharmacy Specialist – is responsible to: 1. Provide comprehensive medication management for critically ill patients as an active member of the interdisciplinary team, participating in daily rounds and prospectively reviewing therapy for appropriate indication, dose, route, interactions, allergies, and adverse drug events. 2. Optimize pharmacotherapy through therapeutic drug monitoring, pharmacokinetic and organ-function–based dosing (renal,hepatic, renal replacement therapy), and assessment of pain, agitation, sedation, delirium, and nutrition support. 3. Perform medication reconciliation at ICU admission, transfer, and discharge, and provide accurate drug information and evidence-based pharmacotherapy recommendations to the critical care team. 4. Respond to medical emergencies and time-critical events (cardiac arrest / code blue, rapid response, sepsis, trauma, shock), providing medication expertise during resuscitation. 5. Lead antimicrobial stewardship and high-alert medication management, supporting appropriate selection, dosing, de-escalation, and monitoring of anti-infectives and other targeted agents within the critical care units. 6. Develop, implement, and evaluate evidence-based critical care protocols, guidelines, order sets, and care pathways aligned with institutional and international standards. 7. Serve as medication-safety leader by identifying, preventing, resolving, and reporting adverse drug events; leading incident investigation, Root Cause Analysis (RCA) and Corrective and Preventive Actions (CAPA); and optimizing medication-safety technology (smart infusion pumps (DERS), clinical decision support). 8. Support safe, efficient, and cost-effective critical care pharmacy operations by ensuring medication availability in collaboration with the inpatient pharmacy and procurement team; standardizing medication concentrations, dilutions, and administration guidelines; and promoting formulary-based medication use. Ensure regulatory compliance and accreditation readiness (e.g., JCI and applicable national requirements) and represent pharmacy on the Pharmacy & Therapeutics Committee and other institutional and critical care committees. 9. Educate and mentor pharmacy students, interns/Trainees/residents, pharmacists, and health professionals; advance critical care pharmacy practice through research and scholarship; and maintain clinical competence, including Board Certification in Critical Care (BCCCP) where eligible. 10.
